Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• What is my copay?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• What are my out of network benefits?
• Is approval required from my primary care physician?
• Do I need an authorization prior to beginning therapy?
• What are the dates that my authorization covers?

Payment
Cash, check, Visa or Mastercard credit cards.. Payment is due at the time of service.

Cancellation Policy
If you are unable to keep a scheduled appointment, please provide 24 hours advanced notice to ensure that you will not be charged for the missed appointment. If less than 24 hours notice is given to cancel an appointment, you will be required to pay the full cost of the session.
